Request a Repeat Prescription

There are several ways in which you can request your repeat prescription.

Please note, a repeat prescription is an item of medication that is listed on your repeat slip. An acute medication is normally a "one off" medication used to treat an acute problem, such as a steroid cream. These medications will not be listed on your repeat slip. All acute medication requests will need to be passed to a GP. The GP may, at their discretion, choose not to prescribe the medication again. You may be asked to come back to see a clinican if this is the case.


Telephone

We have a dedicated prescription line for you to use to order repeat prescriptions. This number is available Monday to Friday between the hours of 10am and 12m.

Online
By far the easiest way of ordering your repeat medication is via the Patient Online system. You will be able to order your repeat medication 24 hours a day via the website or an app on your phone/tablet.

You will need to have registered an account before you can use this service. For more information, please see the Patient Services section.


Drop it in
You are of course able to drop your repeat re-order form into us here at the surgery. There is a box placed in the main entrance. Please tick the items you require on your re-order form and drop it into the box. A new re-order form will be attached to your new prescription.


Send your order to us
If you prefer you can send your prescription re-order form to us in the post. Please ensure you state your Name, Date of Birth and your requested items if requesting by post.

Send to: Reception, Dartmouth Medical Practice, 35 Victoria Road, Dartmouth, TQ6 9RT.

Please be advised that we cannot be held responsible for delays in the post, or lost items.


Fax your order
